Before you begin

Use the form below to continue to your NeuLine Health patient paperwork. Have the patient’s name, HF number, and email address ready before you begin. Enter the patient’s name in the Patient Name field and the HF number in the next field. The HF Number field accepts up to 11 characters.

Enter your email address in both email fields, then check that the two entries match. Review the name, number, and email address for typing errors before selecting “Proceed to Patient Forms.” Fields marked Required must be completed.
